I don't recommend actively pushing link swapping, but if your sites are relevant, its an extra way of getting more traffic. I always link to useful related good quality sites, and they are almost always one way links.

You need to provide all the html and graphics needed to make it as easy as possible for webmasters to take the code and add it to there site.

Make sure they don't add a nofollow to your link.

Give a choice of sizes and colours of graphics.

Encourage natural links

Try and make sure your links dont end up on a page with 30+ links as they may be seen as a link farm

Offer various codes with different anchor text, so everyone isnt using the same anchor text, which could be seen as spam

No not offer rewards for links such as link competitions and renting links.

Link Baiting is perfectly acceptable.
